Overview
Laser hair removal (LHR) using diode laser technology remains a highly effective modality for managing facial hair in female patients. This case involved the use of the Primelase diode laser, currently one of the most powerful FDA-approved and CE marked platforms available, offering safe, efficient, and long-term reduction of hair growth across all Fitzpatrick skin types.
Age: 32
Fitzpatrick Skin Type: IV
Medical History: Polycystic Ovary Syndrome (PCOS)
Primary Concern: Excess terminal hair growth localised to the chin
Hair Characteristics: Thick, coarse, dark hair; presence of some grey hairs in the treatment zone
Clinical Considerations
The presence of grey hairs was noted during the initial assessment. These were not responsive to treatment due to the absence of melanin. As diode lasers target melanin chromophores within the hair shaft, insufficient pigmentation precludes effective photothermolysis.
Device: Primelase Diode (810 nm)
Treatment Area: Chin (with extension to jawline/neck based on growth patterns)
Treatment Frequency: Every 4 weeks
Total Planned Sessions: 12 sessions with subsequent maintenance based on regrowth (typically around 12-week intervals)
Laser Parameters (Skin Type IV):
Patch Test: 30 ms / 9 J/cm²
Session 1: 30 ms / 9 J/cm²
Session 2: 30 ms / 10 J/cm²
Session 3: 30 ms / 11 J/cm²
Session 4: 30 ms / 12 J/cm²
Session 5: 30 ms / 13 J/cm²
Session 6: 30 ms / 13 J/cm²

Pre-Treatment Protocol
Patients were instructed to:
Shave the treatment area within 24 hours prior to appointment
Avoid all topical products (e.g., makeup, fake tan) on the day of treatment
Cease topical prescription actives at least 7 days before treatment
Discontinue any photosensitising medications (e.g., antibiotics) in advance as appropriate
All parameters and contraindications were discussed in a thorough consultation prior to treatment, and a patch test was performed 48+ hours beforehand.
Post-Treatment Care
Immediately post-procedure, aloe vera gel was applied to soothe the area, followed by Obagi SPF50 to protect against UV exposure. Patients were advised to avoid sun exposure for a minimum of 2 weeks pre- and post-treatment and to use a broad-spectrum SPF50 daily.
Outcome After 6 Sessions
The patient demonstrated significant reduction in hair density and rate of regrowth, with noticeable improvement observed even after the first session. This early response had a positive impact, boosting her confidence and giving her a new lease of life – far away from her razor!
Further sessions are ongoing to achieve optimal results.
